Cardiac Risk Factor Modification Program
How the Program Works
Each patient meets with a cardiac nurse practitioner and a cardiologist
who explain the program, give the patient a timeline and nutritional
diary, and schedule any necessary lab work and medical examinations. We
discuss the patient's medical history and any risk factors they may
have, and answer questions about the program. The nurse practitioner and
the cardiologist then collaborate to develop recommendations and a plan of
care.
The individualized plan could include:
- Cholesterol plan/medications
- Nutritional assessment/detailed recommendations.
- Exercise plan/recommendations
- Hypertension management
- Diabetes management
- Smoking cessation
- Alternative medicine therapies
As the only hospital-based program of this kind in Rhode Island, our
program is distinguished by the amount of support we can offer our
patients, the individual attention they receive and excellent
communication with referring physicians.
