Maher, MD. Moderator: We are now ready to take your questions about sports injuries. BigK: Hi, Dr. Maher! Soccer_in_RI: Hello, Dr. I have a question about continuous ankle pain. Dr. Maher: What is your question? Soccer_in_RI: It's concentrated around the Achilles tendon and the heel and my orthopedist says there is no stress fracture, but the pain never goes away. Nothing seems to help. Dr. Maher: What are activities you do that may have contributed to the injury? Soccer_in_RI: I play soccer year-round, both indoor and outdoor, and I play basketball in the winter and coming up this summer I will play also. Dr. Maher: Sounds like an overuse injury with repetitive injuries. It seems like there isn't enough of a rest period for the tissue to heal and you should consider alternating activities. Soccer_in_RI: Thank you, Doctor. none: Hi, Doc. My son pulled out his shoulder playing basketball 2 times. It was put back twice in the emergency room. Following office visits with a doctor and therapy for 2 months, it keeps coming out and now he pushes it back in. What do we do short of surgery? Thanks. Dr. Maher: Physical therapy can be helpful, but if he is young he will probably need surgery to stabilize his shoulder. none: He is 19 years-old. Is surgery the only alternative? Thank you. lorik: Hi, Doc-my question is about patella dislocations... Dr. Maher: Lorik: What is your question about patella dislocations? lorik: I dislocated my right knee when I was 13, and then the other one 6 months ago. I went to PT, but am quite active and still have pain randomly in both. What can I do? Dr. Maher: Sounds like a surgical procedure may be necessary. It can be done arthroscopically in most cases. jiezon: Dr., I recently suffered an Achilles rupture. I have had surgery to repair it. How long does it usually take before I am back on my feet again? Dr. Maher: As the healing progresses, the weight bearing status and activity level progresses. It takes between six months and a year to go back to high-demand activities. Bean: I fractured my collarbone about 4 months ago while snowboarding; now it sticks out quite a lot, is that normal? none: Thanks again, Dr. Dr. Maher: Bean, it can be normal, because there is a lot of new bone that forms around the fracture site. lorik: I also found that my ITB band is irregularly tight... what can I do about that? toni: Dear doctor, I fractured my sacrum and coccyx in a fall while playing tennis. I still have pain-what can I do? Dr. Maher: Lorik, that's a matter of simply stretching the ITB and the muscle that attaches to it, near the hip. lorik: Thanks Doc, Good luck with the rest of the chat....! Dr. Maher: Toni, a doughnut cushion for use when sitting and physical therapy can help. jiezon: I am a marathon runner...is this good news or bad? Dr. Maher: Once you are completely healed, it is certainly possible but go very slowly with training. jiezon: Thanks...I will take it slow... Elena: Dear Dr. Maher, can you please label this problem: I sometimes take a step and my knee just seems to "give out" as if there are not supporting muscles around it. This happens infrequently, but scares me because for those few minutes I cannot walk on that leg at all-I have to hop. Any help would be appreciated, as would any strengthening exercise that you could recommend, too! Thanks. Dr. Maher: Elena, you need to have a thorough examination, including x-rays and maybe an MRI, then treatment will be based on those findings. Elena: Thank you! I've been thinking about that-this gave me motivation to get those x-rays. Thanks again. toni: Dear doctor, can I start to walk a few times a week? Dr. Maher: Walking would be a good way to start exercising. toni: Thanks for the info, Dr. Maher. Bean: I had a "Category 1 Ankle Sprain" it happened about 3 months ago and I still have trouble with lateral movement, even after therapy. What can I do? Dr. Maher: Bean, you should have an MRI to rule out any soft tissue impingement or joint surface damage. Maggie: My son has torn ACL on both legs; he had surgery on one leg and something went wrong. They did more surgery on the same leg about 6 weeks later. This was in early Feb. and March. He is still in terrible pain and cannot get full extension on his leg. They want to do more surgery. Bean: Is "category 1" a bad sprain? Dr. Maher: A Grade 1 sprain is a mild sprain and should heal up within six weeks. Bean: Cool. Thanks. Dr. Maher: There are doctors who specialize in foot and ankle problems. Dr. Maher: Maggie, what is your question? Maggie: He has got a second opinion and has been told that the surgery was done incorrectly. Do you think he should have more surgery? He is in the military and his options are limited. Thank you. Dr. Maher: If his surgery was done incorrectly then he will need a revision procedure to stabilize his knee and allow for full range of motion. Maggie: Thank you. What exactly is a revision procedure? Dr. Maher: That's where the screws from the previous surgery are removed and a new graft is placed with new screws.
