Initial Evaluation Process

Step 1:
Step one includes a medical evaluation, weight and lifestyle evaluation and exercise assessment (which includes a stress test). An appointment for exercise assessment must be scheduled after the medical evaluation.

If you have insurance that covers the medical evaluation, only your co-payment is due at the time of service. If you have insurance that does not cover this evaluation, $125 is due at the time of service.

No insurance will cover the weight and lifestyle evaluation. This is an out-of-pocket expense for all patients. The is a $75 charge that is due at the the time of service.

Please be sure to notify us at least 48 hours in advance if you need to cancel or reschedule the appointments. If an individual no-shows (without any call) the first scheduled appointment, the remaining appointments will automatically be canceled. It will be the individual's responsibility to call the office to reschedule the appointments.

Step 2:
Individuals are required to undergo laboratory tests and an EKG, which are ordered by the Weight Management Program's physician after the medical evaluation (recent lab work or EKG's from your primary care physician may be accepted in some cases).

Step 3:
The exercise stress test is completed only after the individual completes the medical evaluation process (the physician will determine whether the stress test will be conducted at the Center for Behavioral and Preventive Medicine, in The Miriam Hospital, or elsewhere).

Step 4:
Once the individual completes all of the above and meets the criteria for the clinic's Weight Management Program, he/she will be notified by the clinician about enrollment into one of the programs.

The individual will speak to a secretarial staff member to discuss payment of the program treatment charges after the evaluations are complete. A $220 program fee is due at the start of the program. This is a one-time, non -refundable charge for exercise evaluation, visits with the exercise physiologist and nutritionist during the program, use of the exercise facility and patient education materials
