You may be eligible for rehabilitation services if you have a physician’s
referral, and if you’ve been diagnosed with, or recently experienced,
any of the following:
- Angina
- Angioplasty/Stent
- Congestive heart failure
- Heart attack
- Heart valve repair
- Heart transplant surgery

- Heart bypass surgery
- Chronic airway obstruction
- Chronic bronchitis
- Emphysema
- Cystic fibrosis
- Asthma
- Severe leg pain with exertion

Program participants may have access to many of the following programs:

- Diabetes education
- Pain
management
- Lipid management
- Physical therapy
- Respiratory therapy
