At Lifespan Corporate Services, employees not only find rewarding
careers but also have the opportunity to participate in our extremely
affordable and generous benefit program.
Regular employees scheduled to work 20 or more hours per week are
eligible to participate in the following benefit programs:
For most benefits, employees are eligible to participate as of the first
of the month following their date of hire. Others may require a short
waiting period or require approval prior to coverage.
The cost of benefits in most cases is shared between the
employee and Lifespan Corporate Services. The employee's share of the
cost is determined by full-time or part-time employment status, the
coverage options chosen and the number of dependents insured for
medical, dental and/or legal coverage. Premiums are payroll-deducted
either pre- or post-tax (depending on the type of benefit).
Benefit Choices
Medical Insurance
Medical Insurance Coverage Options:

Lifespan Health (Blue Cross and Blue Shield of RI )
When you use a participating provider, there is no deductible,
but co-payments apply to certain services. If you choose a
non-participating provider, most services are covered at 80
percent of reasonable and customary charges after an annual
deductible. Prescriptions are covered in full after a $7
co-payment for generic, $25 for preferred brand and $40 for
non-preferred brand, for up to a one-month supply, at
participating pharmacies. Mail-order maintenance prescriptions
will require a single co-payment of $10.50 for generic, $25 for
preferred brands and $60 for non-preferred brands for up to a
three-month supply.

No Coverage
If you have other medical coverage and choose no medical
coverage through Lifespan, you can receive taxable cash in lieu
of benefits in your paycheck throughout the course of the year.
Full-time eligible employees may receive up to $1,000 annually
to waive medical insurance. Part-time eligible employees may
receive up to $500 annually to waive medical insurance.
Coverage for You and Your Dependents
When you enroll in medical coverage, you can choose coverage based on
the number of dependents you are eligible to cover.

You can cover the following dependents for medical coverage:

Your spouse

Your unmarried dependent children through the calendar year in
which they turn 19 (or 23 if they are full-time students). For
all health programs, if full-time student status ends, coverage
will terminate at the end of the month in which they cease to
qualify as a full-time student.

Adult children between the ages of 19 and 26 are covered until
the first day of the month, following their 26th birthday.

Your disabled dependent children over age 19 who meet plan
eligibility requirements

Your same-sex domestic partner, civil union partner or
common-law spouse
Appropriate dependent verification documentation must be provided prior
to dependent(s) coverage taking effect.
Dental Insurance
Lifespan Corporate Services offers two dental plans which cover a
variety of services, including preventive care and basic services.
Dental Insurance Coverage Options:

DeltaUSA Plan A
Provides 100 percent coverage for preventive and
diagnostic services. Provides 80 percent coverage for minor and
major restorative services and 50 percent coverage for
periodontics.

DeltaUSA Plan B
Provides 100 percent coverage for preventive and diagnostic
services as well as minor and major restorative services.
Provides 50 percent coverage for periodontics, prosthodontics
and orthodontia (for dependent children only/$1,200 lifetime
maximum).

No Coverage
If you choose no dental coverage, you can receive taxable
cash in lieu of benefits in your paycheck throughout the course
of the year. Full-time eligible employees receive $100 annually
to waive dental coverage. Part-time eligible employees receive
$50 annually to waive dental coverage.

Long-Term Disability Insurance
The long-term disability plan replaces a percentage of your income if
you become disabled due to an illness or injury and you're unable to
work for more than 180 days. Employees are automatically enrolled into
the long-term disability program at 60 percent of benefit base salary to
$6,000/month at no cost to the employee.
Employee Life Insurance and
AD&D Insurance
If you should die or become seriously injured in an accident while
you're employed at Lifespan Corporate Services, life and accidental
death and dismemberment insurance could provide you and your family with
an important financial "safety net."
Employees are automatically enrolled at two times annual benefit base
salary at no cost to the employee.
Your supplemental insurance options are based on your annual base pay as
follows:

provided by Lifespan)

4 x annual Benefit base salary (includes 2 x basic coverage
provided by Lifespan)

For those employees who take supplemental coverage, free will
preparation services are available.
Retirement Program
Lifespan's retirement program has been designed as a partnership between
you and Lifespan to help you plan ahead for your future. Lifespan will
work with you, contributing and investing funds for your retirement, to
help you reach your retirement income goals. The retirement program has
three components:

Choice Core Account
Lifespan provides an annual core contribution of 2 percent to 6
percent of annual pay based on years of credited service. You
become eligible to participate on the first day of the month
after you complete one year of service and have reached age 18.
You receive a contribution for each calendar year in which you
are credited with at least 1,000 hours of service.

Vesting
Your own contributions to your account, plus investment
earnings, are immediately vested. Lifespan's core and matching
contributions are fully vested after you have completed three
years of credited service.

Supplemental Tax-Sheltered Annuity Account
This program enables you to reduce pre-tax earnings and
invest your pre-tax dollars in TIAA-CREF. You can choose from 10
investment funds. Lifespan does not match these contributions.
The interest on your contributions accumulates on a tax-deferred
basis.
Paid Time Off
Earned Time Program
Lifespan Corporate Services recognizes your need for flexibility in
using accrued paid time off for absences from work due to vacation,
personal or family illness, or just for a personal day off. The earned
time program allows you flexibility in scheduling time off. Earned time
combines paid time-off benefits including vacation, sick time and
personal days into one bank of earned time. Earned time is flexible,
allowing you to choose how your paid time off benefits are used.
Designated holidays, such as New Year's Day, Christmas etc., are not
part of earned time.
Eligible employees begin to accrue earned time with their first paid
hour of employment. The amount of earned time that an employee is
eligible to accrue is dependent on job category, length of service and
the regular hours of the position. Employees who have successfully
completed 90 days of employment may take accrued earned time with their
supervisor's approval.

You must be regularly scheduled to work at least 20 hours per week to be
eligible for paid holidays. Part-time employees receive pro-rated
holiday time based on standard work hours. If you work on the holidays
listed above, you will be paid a holiday premium in addition to your
regular pay. You may be given time off to observe personal religious
holidays by taking earned time.

The Employee Assistance Program offers resource and referral

Rhode Island Hospital and offers priority placement and

Dependent Care Flexible Spending Accounts
You can contribute, on a pre-tax basis, up to a maximum of

reimburse yourself for eligible expenses such as child and

The dependent life insurance plan pays you a benefit to help

Long-Term Care Insurance
Long-term care is not one service, but many different
services aimed at helping people who have chronic
conditions and are unable to care for themselves.
Services may be needed within the home for day-to-day
activities or in a nursing home when special attention
is needed. Long-term care services are not always
covered by major medical plans or are severely limited
within Medicare and Medicaid.

Education Assistance
Education assistance is available for undergraduate,
graduate and non-degree courses, as well as seminars and on-line
correspondence courses meeting at least one of the following
criteria:

Necessary to fulfill the curriculum requirements
for a degree related to the employee's current
position or another position within Lifespan.
Degree candidates must submit evidence of
acceptance into a degree program by an
accredited college or university.

Necessary to meet the continuing education
requirements to attain or maintain a
professional license or certification related to
the employee's current position or another
position within Lifespan. Certification must be
awarded by a nationally recognized professional
organization.

Necessary to meet the continuing education
requirements of the position as determined by
the department manager if a professional license
or certification is not required.
Reimbursement will be provided according to the following schedule:
Education assistance reimbursement will only be made to employees
actively employed by Lifespan at the time the documentation of the
passing grade or certificate of completion and proof of payment is
provided to the Lifespan benefits department. Requests for education
assistance reimbursement for employees who are on an approved leave of
absence will be made if:
Employees whose standard hours change during the course will be
reimbursed based on the standard hours for their position at the
conclusion of the course. Employees whose standard hours are
involuntarily reduced as a result of a workforce reduction will be
reimbursed based on their standard hours at the time of the start of the
course. The following costs are not covered: books, supplies, travel,
meals, entertainment, late fees, costs not directly related to the
course as well as any costs reimbursable under another subsidy type
program, such as grants, scholarships, etc.
Final grade requirement for reimbursement: "C" or higher, or "Pass" if
on a Pass/Fail grading system, for undergraduate courses. "B-" or
higher, if graduate course. Certificate of completion for continuing
education course.

Relocation Assistance
Individuals who are offered, accept and begin full-time
employment may be eligible for reimbursement towards approved
relocation expenses.

Computer Purchase Discount Program
Employees can purchase computers and related products at
discounted prices.

Legal Services
With legal services you can help protect your rights in some of
life's most frustrating situations. You can protect yourself and
your financial security if expensive legal services ever become
necessary. Legal services participants are also eligible for
financial planning services under the plan.

by insurance, such as vision care, eyeglasses and contact
lenses.

Auto/Home Insurance
You can save on insurance for automobiles, boats, and
home/condo/rentals all through the convenience of payroll
deductions. You can also save more with money-saving discounts
on anti-theft and safety devices, driver safety courses, etc.

HIV Insurance
HIV Insurance provides you a lump sum payment should you
test HIV positive while covered by the plan. The benefit
provides 24-hour coverage and is payable in full at the time of
the claim. If you elect this coverage, the insurance company
will require a confidential, valid, negative test result for
HIV. The test must be performed by a representative from the
insurance company.

Employee Fund
Whether it be a fire that has destroyed your home or a
serious illness that has stricken your child, your fellow
employees want to help. The Employee Fund was created to provide
financial assistance when an unexpected catastrophic event has
left them with nowhere else to turn. Unlike employee loan
programs, money granted through the Employee Fund does not have
to be paid back. The fund is managed by a committee of diverse
employees who determine whether to grant or deny a request based
on established guidelines.

Each Lifespan affiliate offers programs on topics such as
College Planning, Caring for Elders, Balancing Work and Family,
Advance Directives, How to Talk to Your Children About Risk
Taking, Managing the Holidays, Gardening 101, Adoption and
Foster Care, and more. The Human Resources office at Lifespan
Corporate Services welcomes employees' suggestion on topics for
these sessions.
