  • Reporting a Patient- or Visitor-Related Claim

  • If you have reason to believe that a claim may be filed against you and you are covered by the Lifespan Malpractice Plan, you should contact Lifespan Risk Services (LRS) as soon as possible. You may suspect a claim is forthcoming if an attorney requests a copy of your records, you receive a complaint letter, encounter an unexpected outcome, or the patient and/or family are dissatisfied with the care. The hospital Risk Management Department is also available to assist you.

    If the patient/family is represented by an attorney, do not attempt to call the attorney or family. 

    Lifespan Risk Services has its office in Providence (401-444-8273). Each hospital has an on-site risk manager. It is very important that you contact Risk Management before discussing the issue with anyone.
