 Referrals
To refer a child to one of our services, paperwork may be faxed to us at 401-444-4181 or families may call 401-444-5020.
Referrals must include a patient's:
- Name
- Date of birth
- Phone number
- Address
- ICD-9 code
- Physician's signature
Optional pediatric rehabilitation referral pads are available. Please call 401-444-5020 and we will be happy to send one to your physician's office.
