Memory Assessment Program
Our three-part Memory Assessment Program includes:
- A Home Visit
A nurse practitioner will visit you to conduct an evaluation in your
home environment.
- A Visit to Newport Hospital
An appointment is scheduled at Newport Hospital for testing and
medical evaluations by a neuropsychologist and a neurologist. All necessary
testing will be completed on the same day.
While you're being evaluated, a nurse practitioner will meet with
your family and your caregiver to obtain information and compare findings from
the home visit.
- A Care Plan
Within two weeks, a follow-up appointment is scheduled with you and
your family to discuss test results and determine a plan for care. Care plans
are designed to help you maintain independence and quality of life. Your care
plan may include medications, support groups, home care, or a referral to
appropriate agencies such as a Visiting Nurse Service, Meals-on-Wheels, Lifeline (emergency
response service), senior center, or day care program.
Our team approach is used throughout the entire evaluation, with input from your
personal physician, your family, caregivers, the nurse practitioner,
neuropsychologist and neurologist.
Once the plan is in place, the nurse practitioner provides on-going support, and
will periodically check with you and your family to make sure your treatment is
successful.
