Capital Equipment Guide

Inventory
If you would like a print out of all equipment listed under your name as Principal Investigator please contact Leslie Varone at 444-8696 or email her at LVarone@lifespan.org
Tagging
Once the charge has hit your responsibility center, which usually is within a month or two of ordering the piece of equipment, Research Administration will send you a copy of the database record along with the silver identification tag number to be affixed to the piece of equipment. It is your responsibility to make sure that the tag number assigned matches the purchase order number in which it was purchased from. Research Administration at that time may require additional information (e.g., serial numbers, locations, contact name, etc.).
If the silver tag at any time falls off a piece of equipment, please contact Jill Martin at 444-5196. She will issue another tag number to you and changes will be noted in the equipment database.
Re-Inventory
We are required to re-inventory federal capital equipment within 2 years from the last inventory date. Research Administration will send you a copy of the database record along with a re-inventory form which will need to be completed and returned ASAP to Research Administration.
Disposal
Fill out the equipment disposal form and forward it to Research Administration, 3rd floor Aldrich Building. If the piece of equipment is computer related you will need to contact Information Services. Contact Facilities Management for disposal of all other equipment.
Security
If you have a piece of equipment that was stolen, contact the security department and also report it to either Leslie Varone at 444-8696 or Jill Martin at 444-5195.
Transfers
If you are transferring to another institution and considering taking any or all of the equipment listed under your name you need to request this in writing. Contact your Research Administrator.
