Discharge Information:
When patients left the hospital, had they been given written information about what to do during their recovery at home?

The feedback survey asked patients about information they were given when they were ready to leave the hospital. Patients told whether anyone gave them information in writing about symptoms or health problems to lookout for. Patients also told whether anyone had talked with them about the help they would be needing after they left the hospital.

Bars below tell what percent of patients at each hospital said yes, they were given this information before leaving the hospital.
