8. Complaints/Informational Inquiries
If you believe your privacy rights have been violated, you may
file a complaint with the Secretary of the Department of Health
and Human Services, Hubert H. Humphrey Building, 200 Independence
Avenue, SW, Washington, DC 20201. You may also file a complaint
with the Lifespan Privacy Officer at the address and phone number
below.
You will not be penalized for filing a
complaint, nor will you be asked to waive your rights as a condition
of treatment.

or inquires about their protected health information, this Notice,
or our privacy practices, that they would like to pose to officials
