Outpatient Pulmonary Health
and Rehabilitation Program

Eligibility Criteria

You are eligible to participate in the outpatient pulmonary health and rehabiliation program if you meet the following criteria:

• A diagnosis of :
  • Chronic Bronchitis
  • Asthma
  • Emphysema
  • Cystic Fibrosis
  • Pulmonary Fibrosis
  • Asbestosis
  • Obesity
• A referral from your physician.
• A pulmonary function test within the past 6 months that shows moderate to severe irreversible COPD or you meet the medical criteria qualifications for home oxygen coverage.
• You experience significant respiratory symptoms as a result of modest levels of exertion or while performing normal daily activities.
• No other serious or unstable medical conditions.
• The ability to understand and follow educational and instructional materials.
• Motivated to do well in rehabilitation.
• At least three smoke-free months prior to admission into the program
