What to Bring to Your First Appointment
In addition to the completed form, please bring:
- A photo ID and insurance card(s)
- Co-payment (if any)
- Any recent diagnostic images or test results that you may have. (This is only necessary if your testing was not performed by a Lifespan provider.)
Update Your Insurance
Please contact your insurance provider before your first appointment to update the name of your primary care provider. If you do not inform your insurer, we may not be able to see you for your appointment.
