Adolescent Medicine Services: Information for New Patients

To maximize your time with your physician, please download the new primary care patient form (PDF) or the eating disorder program patient form (PDF), print and complete the document at home, and bring it to your appointment.

What to Bring to Your First Appointment

In addition to the completed form, please bring:

  • A photo ID and insurance card(s)
  • Co-payment (if any)
  • Any recent diagnostic images or test results that you may have. (This is only necessary if your testing was not performed by a Lifespan provider.)

Update Your Insurance

Please contact your insurance provider before your first appointment to update the name of your primary care provider. If you do not inform your insurer, we may not be able to see you for your appointment.

MyLifespan Online Patient Portal

Please enroll in our patient portal, MyLifespan, which allows you to request or reschedule an appointment, renew prescriptions, review your laboratory and imaging test results, and communicate with your care team. Get started with MyLifespan »
