Schedule a COVID-19 Vaccine Appointment at Lifespan Urgent Care

Lifespan Urgent Care is now providing the COVID-19 vaccine. To receive the vaccine, simply visit a Lifespan Urgent Care location or schedule an appointment online and avoid the wait.

To receive the vaccine at Lifespan Urgent Care you must be at least 18 years old. Please note that according to Rhode Island Department of Health guidelines, you also must be a Rhode Island resident (or work or go to school in Rhode Island).

You will be required to show proof of age and residence when you arrive for your vaccine appointment. If you do not meet the Rhode Island Department of Health criteria, you will not be able to receive the vaccine when you arrive.

Are you a Rhode Island resident, or do you work or go to school in Rhode Island?

Unfortunately you are unable to receive the vaccine at this time.

At this time, the Lifespan vaccination program is only available to Rhode Island residents.

Are you at least 18 years old?

Unfortunately you are unable to receive the vaccine at this time.

At this time, the Lifespan vaccination program at Lifespan Urgent Care is only available to those who are 18 years old and older.

Have you received another vaccine in the past 14 days?

Unfortunately you are unable to receive the vaccine at this time.  

Please check back to schedule your COVID-19 vaccine 14 days after your last vaccination.

Have you received a Monoclonal Antibody or Convalescent Plasma Infusion in an outpatient clinic or hospital for the treatment of COVID in the past 90 days (3 months)?

Unfortunately you are unable to receive the vaccine at this time.  

Please check back to schedule your COVID-19 vaccine a minimum of 90 days after your infusion treatment.

Do you have a history of a severe allergic reaction, where your throat closed or required emergency care?

If you have had a severe allergic reaction in the past,  you will need to register for a special vaccination clinic with additional patient monitoring. Please continue your registration below with the understanding that it may take additional time to schedule you into a special vaccination clinic.
