Lifespan Employee COVID Vaccination Program

Lifespan has a limited number of COVID-19 vaccine doses available for employees. Please use the form below to sign up for our vaccine waiting list. As additional vaccine doses become available for employees, Lifespan Employee and Occupational Health Services will call you to schedule a vaccine appointment. Remember that you will only receive a call when there is vaccine available.

Please note that vaccine supply is limited and Lifespan employees are encouraged to obtain vaccine from other sources if possible. Please check the Rhode Island Department of Health website for information about available clinics.
