Jennifer Nappi, RN, MSN, CDOE, is the nurse care manager for Women’s Primary Care at the Women’s Medicine Collaborative. A registered nurse, Jennifer earned her bachelor of science degree in nursing from the University of Rhode Island and her master of science in nursing degree at Walden University. She is a certified diabetes outpatient educator and holds a certificate in Women’s Health Care. In her role as nurse care manager, Jennifer serves as a patient advocate and enhances care by empowering patients to take an active role in managing their health. She offers tools to help patients reach health goals, helps individuals manage chronic medical conditions, follows-up after hospital visits to help prevent readmissions, and provides information about disease prevention, home care services and community resources.
