The Transition Consultation Clinic is designed to assist families in managing the complex passage from pediatric to adult care. The clinic is held once a month in the Children's Neurodevelopment Center.

Our clinic does not replace your primary or specialty care providers, but rather assists you in making the leap from a pediatric to an adult health care environment.
Our Team
Our multidisciplinary team will help you and your family create a transition plan based on your personal needs and goals. We provide education and support for the necessary steps to transition to adult care services.
Members of the transition consultation clinic may include:
- Physicians who specialize in transition of adolescents and young adults
- Nurses who specialize in transition
- Resource specialists/transition coordinators
- Family therapy specialists
