Your First Visit to the Transition Consultation Clinic

What To Expect

A first visit lasts up to two hours. During the visit, you and your family will:

  • Meet with a nurse to review medical needs, other services you may receive and medications.
  • Meet with a doctor to review medical history, current issues and have a physical exam.
  • Meet with a resource specialist/transition coordinator to review ongoing opportunities for support in all the areas where life changes as you become an adult: medical, educational and vocational, recreational, personal/social and legal guardianship.

A follow-up visit lasts from 30 minutes to an hour. You will see the team member who can best address the issues raised during your first visit. Please bring any forms or information requested at the end of your first visit.

A final visit lasts one hour. Again, you will meet with the team member best suited to your needs.

What To Bring

  • List of doctors you have seen
  • Medical records and summary
  • List of current medications—if you aren’t certain, please bring all medications in a bag for the nurse and doctor to review
  • Individual education Plan (IEP)
  • Your CEDARR Family Care Plan
  • Questions from both parents and patient

You Will Receive

  • Opportunities for you and your family to speak for yourselves in a medical setting
  • Developmentally appropriate and condition-specific health screenings and guidance
  • A transition plan that outlines the steps needed to transition to adult health care, including developing self-care skills
  • Health care summary
  • An emergency health plan
  • Resource support to help you identify adult doctors, support services and insurance options
