Transitions Clinic is an innovative clinical program that provides primary care and supportive services for medically complex patients who have been recently released from prison

Connect for Health provides screenings at primary care clinics to identify patients' social needs -- such as clothing, food, housing and utility assistance -- and to connect them with available services.

Preventing Hospitalization Following Incarceration

Koren Carbuccia, a community health worker with the Transitions Clinic, accompanies Lisa Vazquez on a medical appointment at the Rhode Island  Hospital Center for Primary Care on Chapman Street. The program pairs recently released and medically complex prisoners with health workers who have once been incarcerated to provide intensive, hands-on peer support.
