Changes Due to COVID-19 Preparedness

Due to the rapidly changing COVID-19 situation, our Jamestown office hours have temporarily changed. Your providers and your health care team remain committed to caring for you during this unprecedented time. We will continue to offer telephone and video visits Monday through Friday. Scheduled face-to-face appointments will be on Wednesdays only as determined by your health care provider. There is additional availability for scheduled face-to-face visits at our Newport location.

About Jamestown Family Practice

The Jamestown Family Practice has been providing comprehensive medical care for children, parents and grandparents since 1984 and offers a knowledgeable and compassionate staff to provide complete care for you and your entire family.

To make an appointment, please call us at 401-423-2616. If you need to speak with one of the physicians or the nurse practitioner, please give your information to the receptionist and your call will be returned as soon as possible.

For your convenience, we have a separate line for prescription refills: 401-423-3114.

When you come in for your appointment, please bring a list of medications and doses you currently take, including over-the-counter medicines and herbal and vitamin supplements.

Our Hours

Our hours are Monday through Friday from 8 a.m. to 5 p.m., with extended hours on Wednesday from 8 a.m. until 6:30 p.m. During weekends, holidays and nights, Newport Hospital family physicians and family nurse practitioners are on call for emergencies.

For routine appointments such as physicals and follow-ups, please call the office to schedule.

Our Providers

Family Medicine Physician

Grace Stephan, MD is a board-certified family medicine physician with experience in urgent care and outpatient medicine. Dr. Stephan earned her bachelor of science degree in biological sciences and biochemistry from the University of Windsor, College of Arts and Science. She went on to receive her medical degree from St. George’s University School of Medicine, in Grenada. Dr. Stephan completed her internship and residency in family medical training at St. John Hospital and Medical Center, Detroit.

Dr. Stephan is a member of the American Academy of Family Physicians and the American Medical Association. 

Family Medicine Physician

Kenneth Wells, MD, is a board-certified family medicine physician at Lifespan Physician Group, Jamestown Family Practice.

Dr. Wells received his medical degree from New York Medical College, Valhalla, NY. He completed his internship at the Naval Medical Center, Portsmouth, VA. He also completed Navy Flight Surgeon Training at the Naval Aerospace Medical Institute, Pensacola, FL. Dr. Wells completed his family medicine residency, serving as chief resident, at the Naval Hospital Bremerton, Bremerton, Washington. 

Dr. Wells is a member of the American Academy of Family Medicine.

Nancy Jones, APRN-CNP

Nancy Jones, APRN-CNP, received her MS in Nursing from the University of Rhode Island. She is certified by the American Academy of Nurse Practitioners as a family nurse practitioner.
