Clinical Pharmacist

Eridania Teixeira, PharmD, provides specialized pharmacy services for the Hepatitis C treatment program and diabetes clinic at Rhode Island Hospital. She participates in a multidisciplinary team and provides medication therapy management services in the hepatology and diabetes clinic through utilization of a collaborative practice agreement for the management of hepatitis C infection and metabolic syndrome.

She is actively involved in the continuation of patient care to prevent medication errors, improve medication adherence and assist in proper management of adverse drug reactions and successful completion of medication therapy. She is also involved in educating pharmacy students and medical residents who attend to patients in both specialties.
