Healthy Transitions

Healthy Transitions is a program designed to assist youth and young adults ages 16 to 25 who are experiencing first episode psychosis (FEP). 

Symptoms of First Episode Psychosis (FEP)

Are you or a loved one suddenly experiencing the following symptoms?

• Disorganized thinking and speech
• Increased confusion
• Withdrawal or social isolation
• Decrease in functioning
• Hallucinations
• Delusions

If so, you or your loved one may be suffering from first episode psychosis (FEP). It can be difficult to let your loved ones know what is going on and ask for help. 

Our Services

Healthy Transitions provides services such as:

• Medication management
• Mental health counseling
• Education and employment support
• Substance use treatment
• Referrals to other resources and programs like vocational rehabilitation and community support
• Coordination of care

We employ coordinated specialty care, a team-based, wraparound approach that seeks to ease the transition to adulthood for those experiencing early psychosis. Our specialists can help you manage your symptoms in a way that works with your lifestyle.

Tailored to your specific goals, our services are designed to help you gain independence in self-managing your illness.

Healthy Transitions is supported by the Rhode Island Department of Behavioral Health, Developmental Disabilities, and Hospitals (BHDDH).

The Healthy Transitions programs serve the Pawtucket, Central Falls, Cranston, Johnston, North Providence, Smithfield, Scituate, Foster and Glocester communities, and all communities in Washington County, RI.
