Advance Care Planning Education

Coastal Medical offers a  free education session for Coastal patients and families that provides information and support for regarding advance care planning and advance directives.  See below for the next available session.

An advance directive is a tool for you to identify and document your wishes surrounding your health goals and preferences at the end of life.  Most importantly, having these conversations with your provider and family early ensures that your wishes are known and honored when you are unable to make decisions for yourself.

The goal of the Advance Care Planning education session is to

  • Define what Advance Directives are in detail  
  • Discuss advantages of completing advance directives 
  • Understand the different types of advance directives and identify the right documents for you
  • Provide detailed steps on how to discuss with your provider and complete the documents to successfully

Chronic Obstructive Pulmonary Disease (COPD) 

Session 1: COPD Basics and Flare Ups 

Learn about the causes and the symptoms of COPD. Understand how COPD affects your body and your COPD care plan. Review the causes of COPD exacerbations and how to control your COPD flare ups. 

Session 2: COPD and Medication 

Learn about COPD medications and treatment options. 

Session 3: Breathing Techniques 

Meet with a respiratory therapist and learn breathing exercises and strategies to manage shortness of breath.

Diabetes

Session 1: Diabetes Basics and Self Management 

Learn about the causes of diabetes and the symptoms of high or low blood sugars. Review self-monitoring of blood sugars & certain factors that may affect your readings. Lastly, learn about how to manage your blood sugars when you are sick. 

Session 2: Diabetes and Nutrition 

Learn how exercise and food can affect your blood sugar level. Learn how to read nutrition labels and what foods are best to keep blood sugars under control.  Learn how to develop your own healthy meal plan. 

All classes will be held online using Zoom. You do not have to have a Zoom subscription to view the classes.

Heart Failure

Session 1: Heart Failure Basics 

Learn about the causes and the symptoms of HF. Understand how HF affects your body and your HF care plan 

Session 2: Heart Failure Medications and Heart Failure Flare Ups

Learn about HF medications and treatment options. Review the causes of HF exacerbations and how to control your HF flare ups. 

Session 3: Heart Failure and Stress Management 

Learn how stress can impact HF and learn strategies to manage stress. 

Session 4: Health Decisions and Planning Ahead 

Discover how to have a voice in your health decisions when you can't speak for yourself. 

Session 5: Heart Failure and Nutrition 

Learn how managing your diet and sodium intake can help decrease your symptoms and prevent a flare. 

Session 6: Heart Failure Activity and ask the Cardiologist 

Meet with a physical therapist and learn how regular exercise can help you better manage your HF. A cardiologist will be available to answer questions you may have on heart failure.

Smoking Cessation

This is a free, 7-week smoking cessation information and support class for Coastal patients wishing to stop smoking. 

  • Understand your smoking habit 
  • Learn how to avoid and alter trigger situations 
  • Learn how to substitute an alternative for cigarettes 
  • Set a smoking stop date and develop a cessation plan
