Advance Care Planning Education

Advance Care Planning Education (Zoom) Classes 

Coastal Medical offers a  free education session for Coastal patients and families that provides information and support for regarding advance care planning and advance directives.  See below for the next available session.

An advance directive is a tool for you to identify and document your wishes surrounding your health goals and preferences at the end of life.  Most importantly, having these conversations with your provider and family early ensures that your wishes are known and honored when you are unable to make decisions for yourself.

The goal of the Advance Care Planning education session is to

  • Define what Advance Directives are in detail  
  • Discuss advantages of completing advance directives 
  • Understand the different types of advance directives and identify the right documents for you
  • Provide detailed steps on how to discuss with your provider and complete the documents to successfully

COPD

Session 1: COPD The Basics

Learn how your lungs work and how they help you breathe. Understand what COPD is and who gets it. Learn what an action plan is and how it can help you manage your symptoms and prevent flares and hospitalizations.

Learn more about your medications and their role in managing your symptoms. Develop your own action plan, ways to eat healthy and stay active. Discover the resources available to help support you.

Diabetes and Diabetes Prevention

Unless stated as "In-Person Session" classes will be held online using Zoom. You do not have to have a Zoom subscription to view the classes. 

Session 1: Diabetes Basics and Self Management

Learn about the causes of diabetes and the symptoms of high or low blood sugars. Review self-monitoring of blood sugars & certain factors that may affect your readings. Lastly, learn about how to manage your blood sugars when you are sick.

2: Diabetes and Nutrition 

Learn how exercise and food can affect your blood sugar level. Learn how to read nutrition labels and what foods are best to keep blood sugars under control.  Learn how to develop your own healthy meal plan.

Dining Out with Diabetes

Join us to learn about making healthy choices when dining out. We’ll talk through tips for navigating everything from restaurants and fast food to even your favorite coffee shops. This session will review topics including label reading and portion sizes, as well as, focus on strategies to reduce carbohydrate and calorie intake while still enjoying your time out.

Diabetes and Navigating the Grocery Store

This class will review strategies for grocery shopping including how to prepare before you go, what to look for when you’re shopping, and tips for choosing healthier alternatives for common food items.

Diabetes and Medications

Join us for a class taught by one of our clinical pharmacists for a tell-all on diabetes and medications. We’ll answer commonly asked questions, debunk some medication myths, and leave time at the end for a Q&A session.

Diabetes and Pattern Management

What is the purpose of checking blood sugar so often? What should I be doing with that information? In this session we'll discuss pattern management and explain how to interpret your blood sugar readings to help achieve your diabetes goals. We will review sample blood sugar logs and continuous glucose monitor reports for a guided discussion on how to take action.

Learn about recent advancements in diabetes technology including continuous glucose monitors, insulin pumps, and smart phone apps. Whether you have type 1 or type 2 diabetes, we’ll talk about how to incorporate these advancements into your own diabetes care plan.

Approximately 1 in 3 Americans have prediabetes. More than 80% of those with prediabetes do not know they have it. Learn the risk factors and how to lower your chances of developing diabetes.

Diabetes Prevention Program (DPP)

The free Diabetes Prevention Program (DPP) at the Lifespan Community Health Institute is an evidence-based program that teaches people at risk for developing diabetes how to implement a healthy lifestyle with the goal of preventing or delaying the onset of type 2 diabetes.  To learn more go to the Lifespan DPP page

This program will start as a virtual class (via Zoom) and move to in-person when permitted. Childcare and transportation assistance available for in-person sessions.

Emotional, Psychological, and Social Well-being

Anxiety Management in Children (ages 3-11): A Parent's Guide

Join us for an overview and panel discussion on different concepts to manage anxiety in children between the ages of 3 to 11. This class is presented by Coastal Medical’s behavioral health clinicians, including our licensed clinical social workers as well as our child and adolescent psychiatrist. Following the presentation, we will answer live questions typed into the chat by the audience.

Being a caregiver requires a lot of attention, all of the time. This discussion will look at the silent signs of caregiver stress, and introduce strategies for efficient and meaningful care and connection.

Grief can come out of many circumstances: the end of a relationship, retirement, leaving a home, or the loss of a pet or loved one. During this conversation we will discuss some of the myths and misconceptions surrounding grief as well as recognizing what might be a grief reaction/response and accessing support.

Let’s get moving! Physical activity is an important piece of your overall health. Join us for a class where we will review the benefits of physical activity and discuss ways to modify an exercise routine based on your needs. We’ll specifically address exercise and discuss SMART goal setting, as well as other topics of overall well-being, including self-care and sleep.

This is a free, smoking cessation information and support class for Coastal patients wishing to stop smoking. 

  • Understand smoking habits 
  • Learn how to avoid and alter trigger situations 
  • Learn how to substitute an alternative for cigarettes 
  • Set a smoking stop date and develop a cessation plan
