Primary Care at the Adolescent Healthcare Center

The Adolescent Healthcare Center provides primary care for young people ages 10 to 21. We work closely with patients to help them manage their health care and reach optimal wellness.  We believe supportive adults are important to help youth achieve well-being, and we include them in visits and care planning when appropriate and with our patients’ knowledge and consent.

We are a Patient Centered Medical Home (PCMH), which means we use a model of care that puts patients and their families at the forefront of care. PCMHs build relationships between people and their health care teams. Your care team is responsible for ensuring they work with you to provide you with the best quality care possible.

Services at the Adolescent Healthcare Center

Our services include:

  • Well visits (checkups), including sports physicals
  • Follow-up visits for chronic problems
  • Sick or urgent care visits
  • Mental health care, including medications and short-term therapy
  • Social work counseling
  • Immunizations/vaccines
  • Nutrition counseling
  • Coordination of care with community agencies
  • Reproductive health care for males and females
  • Gynecologic care including Pap tests and issues related to menstruation
  • Birth control and family planning
  • Pregnancy testing and options counseling
  • Screening, testing and treatment of sexually transmitted infections (STI)
  • Prevention of STIs, including condoms and an HIV prevention strategy of pre-exposure prophylaxis (PrEP)
  • Comprehensive health care services for lesbian, gay, bisexual and transgender youth
  • Guidance for parents and counseling for youth about puberty
