Community Support Services

Gateway Healthcare’s Integrated Health Home (IHH) and Assertive Community Treatment (ACT) programs provide coordinated care that treats the whole person by including primary care, specialist care, and behavioral health together. Our mission is to help people reach a level of recovery that enables them to lead productive, independent lives.

Services include:

  • Supportive counseling, advocacy, encouragement, and assistance in accessing resources
  • Referrals to internal resources (for substance abuse treatment and psychotherapy)
  • Rehabilitative services
  • Crisis intervention
  • Nursing services
  • Daily living skills training
  • Evaluations of responses to treatment interventions
  • Ongoing needs assessments

Who is Eligible?

To be accepted into the IHH or ACT programs, clients must have an eligible diagnosis, such as:

  • Schizophrenia (as well as unspecified schizophrenia spectrum and other psychotic disorders)
  • Schizoaffective disorder
  • Bipolar disorder
  • Recurrent major depressive disorder
  • Obsessive-compulsive disorder
  • Borderline personality disorder

We review clients’ medical history and standardized assessment of daily living activities and refer them to either Integrated Health Home (IHH) services or the Assertive Community Treatment (ACT) team. These services help reduce health care costs by avoiding hospital admissions or readmissions and unnecessary emergency department visits.

Our Programs

Integrated Health Home (IHH) services:

  • Follows the model of the patient-centered medical home (PCMH)
  • Links clients to other community supports and enhances coordination of medical and behavioral health care for persons who have co-existing chronic illnesses
  • Ensures the client’s stability and community tenure
  • Monitors and provides medical, social, educational, or other services necessary to meeting basic human needs

Assertive Community Treatment (ACT):

  • Serves clients who struggle with substance use disorders and housing issues and who are underemployed or unemployed, or who have been unsuccessful with less-intensive treatment models
  • Coordinates services from several specialty areas to provide rehabilitation, treatment, and support
  • Tailors the treatment plan to the client, and provides 75 percent of services in the community
  • Reduces hospitalization, increases housing stability, and improves quality of life for people with severe symptoms of mental illness.
