Promoting the Integration of Primary and Behavioral Health Care (PIPBHC) for Children and Families

PIPBHC is a community health program that helps families bridge the gap between physical and mental health. PIPBHC services are centered on the entire family’s health not just the individual client. We will coordinate and treat physical and behavioral health conditions and/or substance use issues of clients. Additionally, PIPBHC offers a variety of treatments and referral options to clients and their family to best manage their symptoms and health conditions. Our goal is to use evidence-based practices and collaboration to improve overall family functioning and wellness. We also assist families in accessing services that address barriers related to housing, social connectedness, childcare, education, employment, food security, and income/benefits.

Eligibility for the PIPBHC Program

Adults and children are eligible if they:

  1. Live in South County (Washington County)
  2. Live in a household with a child under 18, or someone who is pregnant
  3. Receive primary or behavioral health care from a local community health center 
  4. Have a physical condition and mental health issues, or have difficulty with substances including tobacco use

PIPBHC offers the following services:

  • Care coordination and case management services 
  • Focused treatment and interventions through telehealth and in-person
  • Health education with our nurse
  • Parenting supports
  • Children and teen groups
  • Substance use treatment and coordination
  • Tobacco cessation
  • Access to mental health services
  • Online self-monitoring surveys with your care team
  • Access to funding for health activities and approved co-pays
  • Goal attainment program to address chronic conditions and enhance health
