Pediatric Primary Care Services That Matter

Hasbro Children’s Hospital Pediatric Primary Care enjoys a long history of promoting the health and well-being of children. We are nationally recognized by our National Committee of Quality Assurance (NCQA) certification and by the State of Rhode Island as a pediatric primary care medical home through PCMH-Kids, providing a high-quality family- and patient-centered medical home for children and youth.   

We provide optimal health and development through a commitment to quality measurements, accountability for costs and outcomes, a focus on population health, and dedication to data-driven system improvement. 

Pediatric Primary Care prioritizes team-based care, with families being at the center.

Our team of pediatric providers includes supervising physicians paired with a group of resident physicians along with nurse practitioners, nurses, medical assistants, outpatient service representatives, interpreter services, a social worker and lactation consultant.
 
Each child will have their own pediatrician and team for more personalized care.  We will coordinate care both inside and outside the practice.

Pediatric Primary Care Clinical Programs

We provide primary care, and the care coordination and advocacy work necessary to safely care for patients with multiple diagnoses, technology dependence, and significant functional limitations.  We communicate with their subspecialty providers and provide support to patients and families.

Connect for Health

A program available to all patients of Pediatric Primary Care Clinic that provides services to help you be healthy. These include:

  • Getting diapers, formula or clothing for your family
  • Applying for health insurance for yourself or your children
  • Finding after school activities or affordable childcare for your children
  • Paying utility bills
  • Legal assistance with housing, immigration, or guardianship/custody issues
  • Worries that your home is unhealthy or you may become homeless
  • Finding job training, GED or English classes

Fostering Health Program

The program was developed in 2014 to better address the special needs of children in foster care, kinship care and congregate care.  Our team includes a pediatrician, nurse practitioner, and a nurse.

The Fostering Health Program’s mission is to provide a timely, comprehensive, family-centered medical home to any foster child who does not have a pre-existing primary care provider.  Specifically, we aim to provide:

  • Initial intake evaluation within 1 month of removal from home or transition of placement.
  • Comprehensive evaluation including mental and developmental screening.
  • Collaboration with our partners including Department of Children Youth and Families caseworkers, foster care families, residential treatment staff, birth families, mental health providers, educational advocates, subspecialty providers and schools.
  • Ongoing primary care that is tailored to meet the needs of the individual child and family.
  • Education and support to all those involved in the care. 
  • Safe and nurturing environment for the foster child to thrive.

HEALTH (Healthy Eating Active Living Through Hasbro)

The mission of the program is to instill healthy lifestyle changes that will benefit the child and family for life.

Children will:

  • Get personalized advice on making healthy changes.
  • Play games to learn healthy habits.
  • Get a free pedometer to help track steps.
  • Rewards for completing home activities, such as frisbees and jump ropes.
  • Learn portions and ways to eat more variety by playing the Healthy Helping game.
  • Get a personal HEALTH Family Coach.
  • Regular follow up to make sure you stay on track with your goals.

Refugee Health Program

Refugee children have specific medical and mental health challenges.  There are barriers to health care access and we aim to develop strategies to overcome these barriers and address specific health needs using the medical home model.  Refugee children are seen within 30 days of settlement to Rhode Island.  They receive ongoing primary care with the same provider with triage for oral health and integrated behavioral or mental health.  There is also a civil surgeon on staff to help with Green Card applications.

Please enroll in our patient portal, MyLifespan, which allows you to request or reschedule an appointment, renew prescriptions, review your child’s laboratory and imaging test results, and communicate with your child’s care team.
