How to Enroll Your Child or Teen in MyLifespan
MyLifespan for your child or teen allows you to be more active in their care, while still protecting their privacy.
Now, parents can follow the steps below to remain up-to-date on their child’s medical conditions, view and make appointments, review laboratory and imagining results, securely communicate with doctors and care teams, manage health information, and much more.
If your child is 11 years old or younger
You can request a MyLifespan account for yourself and an account for your child, with you as a proxy. As their adult proxy, you will have full access to their MyLifespan account.
If your child is between the ages of 12 and 15 years old
You can request a MyLifespan account for yourself and an account for your child or teen, with you as a proxy. This will give you access to portions of their health information, and is designed to protect the confidentiality of your teen’s health information. At 12 years old, your child or teen can also create their own MyChart account, with your consent.
If you have a teenager who is 16 or 17 years old
At 16 years old, your teen can have their own MyChart account without parental/guardian consent. They can choose within MyLifespan if they would like you to have full or limited access.
To ensure the security of your child or teen’s health information, and to ensure their privacy, you must read and agree to the below applicable information prior to requesting a MyLifespan portal on their behalf:
- MyLifespan proxy request form for 0- to 11-year-old patients (PDF)
- MyLifespan limited proxy request form for 12- to 15-year-old patients (PDF)
Next, simply call your child or teen’s provider’s office to setup a MyLifespan portal for them and a proxy for yourself. Depending on which office you are calling, you may be directed to a central pediatric/teen MyLifespan sign-up hotline.
Note that 16+ year old teens can call their provider directly to obtain MyLifespan access; they may elect to grant a limited proxy as well.
If you have any questions, your health care provider can assist you. For additional questions please contact the patient portal customer service team at 401-606-6957 (606-MYLS).
