Diabetes Prevention Program

Are you among the one-third of adults at risk of developing Type II Diabetes? If yes, this free, in-person Diabetes Prevention Program offered by the Lifespan Community Health Institute and the Tri-County Community Action Agency will be offered every Thursday from 5:30 to 6:30 p.m. from May 23, 2024 through April 10, 2025. Enrollment ends on Thursday, June 13, 2024.
The Diabetes Prevention Program is a group-based, facilitated, lifestyle change to prevent or delay the disease.
Transportation assistance and childcare will be available.
Register
This program is free, but registration is required:
- Priority will be given to Tri-County Community Action Agency’s Health Equity Zone (HEZ) communities and to Medicaid beneficiaries.
- Registrants must be 18 years of age to participate.
- Registration closes on June 13, 2024.
